Dental Benefits
Dental coverage is included in your health plan for members until the end of the month in which the member turns 19.
Kaiser Permanente offers an optional dental insurance plan to adults, which includes those individuals whose eligibility for pediatric dental services has ended. This optional coverage is available for an additional charge. Our optional adult dental coverage is underwritten by KPIC (Kaiser Permanente Insurance Company), a subsidiary of KFHP (Kaiser Foundation Health Plan, Inc.), and administered by Delta Dental of California. Once enrolled, you can’t cancel this dental coverage without also canceling your health plan coverage, except during open enrollment or a special enrollment period.

Pediatric dental coverage is included in your health plan for members until the end of the month in which they turn 19. We also offer a selection of optional dental plans for adults 19 and older for an additional cost.
Dental plans are administered by Liberty Dental Plan with a business address at 340 Commerce, Suite 100, Irvine, CA 92602.
Individual and family dental plans
Kaiser Permanente’s Individual and Family dental plans offer a robust network with no need for referrals for specialist procedures. There are no waiting periods, so members enjoy comprehensive dental benefits on Day One.
In addition, each of KP’s adult dental offerings can be paired with optional enhanced cosmetic dental benefits, including full orthodontic services, implants, and more. If purchased, these orthodontic benefits cover all family members, including children.

In Maryland, you can enhance your KP medical coverage with one of the following dental options, available for purchase as riders (open to those age 19 and older).
Kaiser Permanente Smile KPIF Dental EPO
This plan offers the convenience of predictable costs, no deductibles, and no overall limit on annual benefits. Members pay a copay only for services that they receive – there is no charge for preventive care office visits. The plan includes comprehensive coverage of the full range of common dental procedures, including preventive, basic restorative, and major restorative services. Additional coverage for enhanced cosmetic orthodontic procedures can be purchased through the Kaiser Permanente OrthoPlus rider.
This plan is the most like the Adult Dental HMO plan offered for 2023 through Dominion Dental.

Kaiser Permanente Smile KPIF Dental PPO Basic
This plan gives members access to a network of high-quality in-plan providers, plus the freedom to see any licensed dentist outside of the plan. With in-plan dentists, members receive 80% coverage (member pays 20%) for preventive care procedures that include the services most frequently performed for adults. If members choose to visit a licensed dentist not included in the network of participating dentists, they receive 60% out -of-network coverage for preventive services.
The plan also includes coverage of basic restorative and major restorative services. Additional coverage for enhanced cosmetic orthodontic procedures can be purchased through the Kaiser Permanente OrthoPlus rider. The maximum annual benefit covered is $1,000, both in-plan and out-of-plan, and there is a $50 deductible per person.

This plan offers a richer alternative to the KP Smile KPIF Dental PPO Basic plan. It gives members access to a network of high-quality in-plan providers, plus the freedom to see any licensed dentist outside of the plan. With in-plan dentists, members receive 100% coverage (member pays nothing) for preventive care procedures that include the services most frequently performed for adults. If members choose to visit a licensed dentist not included in the network of participating dentists, they receive 80% out -of-network coverage for preventive services.

Additional coverage for enhanced cosmetic orthodontic procedures can be purchased through the Kaiser Permanente OrthoPlus rider. The KP Smile Kids KPIF Embedded Dental EPO provides dental benefits for children aged 18 and younger. It has predictable copays and no overall limits on annual benefits. Deductibles may apply depending on the provisions of the medical plan.
The plan includes comprehensive coverage for preventive, basic restorative, and major restorative services, with no need for referrals. It also includes a medically necessary orthodontic benefit. This can be enhanced with an optional cosmetic orthodontia rider that provides coverage for braces and other procedures, if purchased with an Adult dental plan.

Cosmetic Orthodontic Benefits
Medically necessary orthodontic coverage for children under age 19 is included in Kaiser Permanente’s health plans. This covers conditions such as cleft palate if associated with a medical diagnosis, with a referral from your Primary Care Physician.
Enhanced coverage for cosmetic dental procedures including implants, veneers, and orthodontic treatments like braces, is available for purchase through the Kaiser Permanente OrthoPlus rider, at an additional charge. When purchased with an adult dental plan, this rider covers all family members including children under age 19. There is a lifetime benefit limit of $1,000 per individual. Dental plan is underwritten by Dominion Dental Services, Inc. ("Dominion Dental"), a separately licensed stand-alone dental carrier with a business address at 251 18th St S Suite 900, Arlington, VA 22202.

Dental Benefits
You can choose to add dental coverage from Delta Dental for an additional monthly charge. An adult/family plan is available for adults and dependents 25 and younger. To cover children only, a pediatric plan is available for family members 18 and younger. For information about dental benefits and costs, please review your enrollment materials.

Under the Affordable Care Act, pediatric dental coverage is required. If your application includes children 18 and younger and you don’t enroll them in our pediatric dental plan, we’ll contact you to submit an Attestation of Pediatric Coverage with proof of other pediatric dental coverage.

Dependents under 19 must have dental coverage. You can purchase a pediatric dental plan at an additional monthly premium through Kaiser Permanente or another company. There is no option for adding adult dental coverage.

If you enroll in a KPIF (Kaiser Permanente Individual and Family Plan), then by law, you must also enroll in a separate pediatric dental plan. Or, if you already have other pediatric dental coverage that is certified by the health benefit exchange, you must let us know.
